Description
Emerging from the distal trachea at the carina, the right main (principal) bronchus descends inferiorly and laterally toward the hilum of the right lung. Its lumen is supported by discontinuous hyaline cartilage plates, while annular ligaments bridge adjacent cartilaginous segments and maintain airway patency between the more rigid rings and plates. Posteriorly, the membranous wall and trachealis region transition into the bronchial smooth muscle layer as the airway approaches the pulmonary root. Short and wide. Right-sided bronchial anatomy matters because the right main bronchus diverges more vertically from the trachea than the left, aligning with the path an aspirated foreign body often follows in adults. The carina also functions as a procedural landmark during bronchoscopy and endotracheal intubation, and thickening or distortion at this bifurcation can signal peribronchial tumor, lymphadenopathy, or inflammatory change.
