Description
Seen from a superior aspect, the trachea bifurcates at the carina into the right and left main (primary) bronchi, with the last C shaped tracheal cartilages stacked superior to the branching point. The right main bronchus courses inferiorly and laterally with a shorter, wider caliber, while the left main bronchus angles more inferolaterally beneath the expected position of the aortic arch. Posteriorly, the membranous wall of the trachea and proximal bronchi completes the rings, a key contrast to the open posterior ends of the cartilages. A superior view of the main bronchus matters because it maps airway geometry at the carina, where endotracheal tube position, aspiration patterns, and bronchoscope navigation are determined. The steeper, more vertical right principal bronchus explains why aspirated foreign bodies and gastric contents more often enter the right lung, and why inadvertent right mainstem intubation produces unilateral breath sounds and left lung atelectasis. The carina is also a landmark for malignancy and lymphadenopathy assessment during bronchoscopy, and blunting or deviation can signal subcarinal pathology.
