Seen from the medial aspect, the inferior pole (polus inferior renis) tapers into the lower margin of the kidney as the medial border approaches the renal hilum. Just superior and slightly anterior to this pole, the renal pelvis gathers the major calyces and narrows into the ureter at the ureteropelvic junction, while the renal vessels sit more anterior within the hilum in typical anteroposterior order. Posteriorly, the convex parenchymal surface rounds away toward the posterolateral border. Clean landmarks. This medial perspective matters because the lower pole and proximal ureter define the geometry of renal drainage, the exact area where ureteropelvic junction obstruction produces hydronephrosis with calyceal dilatation and lower pole parenchymal thinning. For endourology and stone disease, the infundibulopelvic angle and the relationship of the lower pole calyx to the renal pelvis help explain why inferior pole calculi often clear poorly after extracorporeal shock wave lithotripsy and may instead require ureteroscopy or percutaneous access. Surgeons also reference this region when planning pyeloplasty, where the proximal ureter must be mobilized without compromising segmental blood supply.
