Description

Medial inspection of the posterior segment of a kidney exposes the convex renal capsule draped over dense cortical substance, with cortical tissue continuing into medullary pyramids whose apices form papillae directed toward the renal sinus. The renal columns (of Bertin) lie between adjacent pyramids, and the posterior parenchyma sits posterior to the renal pelvis and major calyces even when those collecting structures are only partially opened in a strict medial view. Superiorly, the outline approaches the suprarenal impression, while the medial border tracks toward the hilum where the renal vein lies anterior to the renal artery, and the pelvis is most posterior. Cortex dominates the outer perimeter. For teaching, this angle is useful when you need to separate parenchymal layers from the collecting system, since a medial view clarifies how cortical thickness relates to the underlying medulla and why papillary tips become the focal point for urine outflow and obstruction. Clinically, posterior segment anatomy matters in percutaneous nephrolithotomy and nephrostomy, where posterior calyceal access aims to traverse a relatively avascular plane near Brodel’s line while avoiding segmental arteries that course within the renal sinus and along the pyramids. It also frames nephron loss patterns: cortical thinning in chronic kidney disease versus medullary involvement in papillary necrosis. Use this artwork in renal anatomy and urogenital block lectures, radiology teaching files paired with coronal CT or IVU-style diagrams, and surgical education materials on posterior calyx puncture planning and segment-preserving partial nephrectomy margins. Anatomical accuracy verified by SciePro's Medical Advisory Board.
