Description

Medial inspection of the superior renal segment brings the upper pole of the kidney into relief, with the thick renal cortex and medullary pyramids of the superior parenchyma tapering toward the renal sinus. Along the medial border, the concavity of the hilum region is implied, where the renal pelvis and major calyces would lie deep to the parenchyma even if not fully opened. Superiorly, the contour approaches the suprarenal (adrenal) impression, while the parenchyma extends inferiorly toward the interpolar region. Orientation is clear: medial is toward the renal sinus, lateral toward the convex renal margin. This angle matters when you teach or plan around segmental anatomy and polar access, because the superior segment is the territory most often traversed during upper pole percutaneous nephrolithotomy and targeted during segmental artery clamping in partial nephrectomy. Short. A medial view also helps explain why upper pole calyces and infundibula can be challenging in flexible ureteroscopy, and why injury at the hilum can threaten collecting system continuity as well as segmental perfusion.
