Description

Prominent in lateral profile, the superior pole of the kidney forms a rounded cranial apex that continues inferiorly into the more vertical lateral border of the renal contour. Closely applied to its superomedial aspect sits the suprarenal (adrenal) gland, separated by a thin plane of perirenal fat within the renal fascia (Gerota fascia). Deep to the capsule, you would expect the upper portion of the renal cortex and medulla to occupy this region, with the renal hilum remaining medial and not the focus from this side-on perspective. Clinically, the superior renal pole is a key anatomic neighborhood because it lies immediately inferior to the diaphragm and posterolateral ribs, with the pleura descending near the upper pole, a relationship that matters during posterior approaches and percutaneous nephrostomy planning. Upper pole renal masses, calyceal diverticula, or staghorn calculus extension can alter the pole’s contour and displace the adrenal, and distinguishing renal from suprarenal origin is a recurring problem in cross-sectional imaging. Small distance, big consequences. Use this view for teaching renal surface anatomy in gross anatomy and urologic surgery courses, for figure plates in radiology texts explaining upper-pole lesions on CT and ultrasound, or for patient-facing education around nephrectomy where the adrenal-sparing plane must be understood.
