Description

Oriented in posterior anatomical position, the superior segment of the kidney occupies the upper pole, where the renal cortex forms a continuous peripheral cap over the medullary pyramids. Superiorly the renal capsule rounds toward the diaphragmatic surface, while the medial border approaches the renal sinus and renal hilum region, where the pelvis renalis, renal vessels, and proximal ureter would lie anterior to the posterior parenchyma. Laterally, the reniform contour thickens toward the convex border, and inferiorly this segment blends into the interpolar parenchyma without a discrete external demarcation. A posterior emphasis on the upper pole matters because the superior segment is commonly supplied by a distinct segmental branch of the renal artery, a pattern that informs partial nephrectomy planning and selective arterial clamping. Upper pole masses are also where you must think about the relationship to the diaphragm and pleura (and on the right, proximity to the liver), since posterior approaches and percutaneous access can traverse the 11th to 12th rib interspace and risk pleural violation. Small margins matter here. Segmental thinking also clarifies why an infarct or embolus can produce wedge-shaped cortical necrosis confined to one pole.
