Description

Spanning the retroperitoneal space, the posterior surface of the kidney is presented with the smooth renal capsule forming the outer contour and the convex lateral border sweeping inferiorly toward the lower pole. Medially, the concave border corresponds to the approach to the renal hilum, where the renal pelvis and proximal ureter would lie anterior to the posterior plane of the kidney, and where the renal artery and renal vein course toward the midline. Superiorly, the upper pole sits in close relationship to the suprarenal (adrenal) gland, while the inferior pole projects toward the iliac fossa and the ureter’s descent. Posterior renal surface anatomy matters because dorsal access is how you reach the kidney in many percutaneous and flank approaches, where the puncture tract traverses the posterior abdominal wall before entering the renal parenchyma and, ideally, a posterior calyx. Needle trajectory and safe windows are judged against the kidney’s posterior convexity and its medial orientation toward the hilum. This is the logic behind posterior calyceal access in percutaneous nephrolithotomy and why posterior subcapsular hematoma and perinephric collections are taught in the same breath as renal capsule boundaries. Use this posterior view when teaching retroperitoneal relationships in gross anatomy or urology modules, illustrating nephrectomy or nephrostomy approach planning in surgical texts, or clarifying renal capsular anatomy in radiology captions for CT or ultrasound correlation.
