Cut across the ventricles in transverse section, the fibrous membranous part of the interventricular septum appears as a small, pale plate at the superior margin of the muscular septum, immediately inferior to the aortic root. Superior and anterior to it lies the left ventricular outflow tract, while the right ventricular outflow tract sits more anterior and to the right; the septal leaflet of the tricuspid valve approaches it on the right-sided, atrioventricular aspect. Continuity with the aortic valve annulus and adjacent fibrous skeleton of the heart is implied, placing this membrane at the junction where ventricular septum, aortic root, and atrioventricular septal structures converge. Tiny structure. Big consequence. This specific segment matters because it is the commonest site of congenital ventricular septal defect (perimembranous VSD), where a small opening can create a left-to-right shunt directed from the higher-pressure left ventricle toward the right ventricle or right ventricular outflow tract. The atrioventricular conduction axis (bundle of His) courses in close proximity to the membranous septum before dividing into bundle branches along the septal endocardium, so device closure or surgical patch repair risks iatrogenic complete heart block. Surgeons also reference this region when working near the aortic valve and tricuspid valve, since misjudging the fibrous boundaries can compromise valvular competence or injure the conduction tissue.
