Description
Positioned on the left side of the interventricular septum, the membranous septum appears as a thin, fibrous segment immediately inferior to the aortic valve and superior to the muscular interventricular septum. From a lateral perspective you can appreciate its proximity to the left ventricular outflow tract, with the aortic root lying superior and anterior relative to the septal surface. The adjacent atrioventricular septal region sits superior and posterior, where the fibrous skeleton links the aortic annulus to the central fibrous body. Small area, big consequences. Clinically, this is the commonest site of ventricular septal defect, and its relationships explain the conduction risks: the atrioventricular bundle penetrates the membranous septum near the right fibrous trigone before coursing along the interventricular septum, so perimembranous VSD closure and aortic valve surgery can produce complete heart block if sutures stray too close. The left-sided lateral view also clarifies why aortic cusp prolapse and aortic regurgitation may accompany perimembranous defects, given the immediate subaortic position and shared fibrous continuity at the aortic annulus. Use this plate in cardiothoracic anatomy teaching blocks on cardiac septation, in pathology texts illustrating perimembranous VSD morphology, or in operative planning materials describing the subaortic septal region during aortic valve replacement or VSD patch repair.
