Description
Arising from the interventricular septum and adjacent trabeculae carneae, chordae tendineae spuriae traverse the cavity of the right ventricle as thin fibrous bands that do not insert on the tricuspid valve leaflets. From an anterior cross-section, these false chords course obliquely from the septal wall toward the free wall, often intersecting the coarse trabecular network in the apical third of the chamber. The septomarginal trabecula (moderator band) lies as a thicker muscular bridge running from the septum to the anterior papillary muscle, providing a positional landmark against which the finer spuriae strands can be distinguished. This sectional perspective matters because right ventricular chordal variants are a recurring source of confusion on echocardiography, cardiac CT, and MRI, where a spuriae band can mimic a thrombus, a ruptured tricuspid chorda, or an intracavitary mass. The relationship to the septomarginal trabecula is the practical differentiator: the moderator band is muscular, contains part of the right bundle branch conduction pathway, and has a predictable septum-to-free-wall trajectory, while spuriae chords are slimmer, more variable, and non-valvular. Clear separation of functional chordae tendineae from non-functional intracavitary strands also supports accurate interpretation of right ventricular remodeling in pulmonary hypertension and congenital heart disease, where trabeculation and chordal bands may appear exaggerated.
