Description

Running from the interventricular septum toward the anterior papillary muscle, the moderator band (septomarginal trabecula, trabecula septomarginalis) is presented in sectional orientation within the right ventricle. An anterior profile logically includes the right ventricular free wall anteriorly and laterally, the septal surface medially, and the trabeculated apical myocardium inferiorly. The band traverses the ventricular cavity as a distinct muscular bridge, separating the inflow region from the more heavily trabeculated outflow-adjacent compartment, with the septal attachment proximal and the papillary insertion more distal toward the ventricular wall. Appreciating the moderator band’s course matters because it commonly carries the right bundle branch as it reaches the anterior papillary muscle, making it a consistent landmark for correlating right ventricular anatomy with conduction. On echocardiography it can mimic an intracavitary mass or thrombus, and a true-to-form sectional view helps separate normal septomarginal trabeculation from pathology.
