Description
Arising posteriorly from the inferior lobar (lower lobe) bronchus of the left lung, the superior segmental bronchus (B6) courses dorsally toward the apical portion of the lower lobe. From this posterior perspective, the bronchial lumen and its early segmental branching sit deep to the pleural surface, with the bronchus positioned medial to the lower-lobe parenchyma and directed superiorly toward the posterior thoracic wall. Adjacent segmental pathways of the basal bronchi would lie inferior and more anterior by comparison, while the lung apex and cupula of pleura remain superior to the lower lobe field. Clean, posterior orientation. This posterior view matters because B6 is a frequent target in diagnostic bronchoscopy and in anatomical segmentectomy of the superior segment of the left lower lobe, where correct identification of the segmental bronchus reduces the risk of wrong-segment resection. Posterior segments also behave differently in aspiration and secretion pooling when patients are supine, and understanding the dorsally directed airway anatomy helps explain recurrent posterior lower-lobe atelectasis or infection patterns. It also mirrors what you see when approaching the hilum from a posterior thoracoscopic corridor.
