Description
Arising above the level of the first rib, the apices of the right and left lungs are shown from an anterior perspective as dome-shaped superior extremities (extremitas superior pulmonis), capped by the cervical pleura (cupula pleurae). Each apex projects into the root of the neck, lying deep to the clavicle and first costal cartilage, lateral to the trachea in the midline and superior to the remainder of the upper lobes. Medially, the apical pleura approaches the thoracic inlet, while laterally it conforms to the inner surface of the thoracic wall. Clear sidedness and symmetry are emphasized. Apical anatomy matters because it is where thoracic and cervical spaces overlap, and small errors in surface landmarking can have real consequences. A Pancoast (superior sulcus) tumor arises at the lung apex and may extend into the brachial plexus and sympathetic chain, producing shoulder and arm pain with Horner syndrome rather than pulmonary symptoms. The cupula pleurae also sits in the path of subclavian venous access and low internal jugular puncture, so this view supports safe needle trajectory planning and explains why an apical pneumothorax can follow misplaced lines.
