Posteriorly, the splenic extremity (splenic pole, extremitas splenica) occupies the left hypochondrium, draped against the posterolateral abdominal wall beneath ribs 9 to 11. The visceral surface faces anteromedially toward the gastric impression and hilum, while the diaphragmatic surface turns posterolaterally toward the diaphragm. Superiorly the pole approaches the fundus of the stomach, and inferiorly it tapers toward the splenic flexure of the colon, with the left kidney lying more medial and posterior in the retroperitoneum. Relationships matter here. A dorsal or posterior definition helps clarify why the spleen is a frequent casualty in blunt left thoracoabdominal trauma, where fractures of the lower ribs can lacerate the thin splenic capsule near its posterior aspect. For operative planning, this orientation also tracks the path of the splenic vessels along the superior border of the pancreas to the hilum and explains why mobilization often begins by dividing the splenorenal and splenophrenic attachments from a posterolateral approach. The posterior pole is also a landmark when teaching referred pain patterns, including left shoulder pain via diaphragmatic irritation (Kehr sign) in hemoperitoneum.
