Description
Posteriorly oriented, the convex facies diaphragmatica of the spleen is presented as a continuous dome that mirrors the left hemidiaphragm, with its maximal breadth positioned superiorly under the costal arch and tapering inferiorly toward the splenic pole. The outer aspect curves laterally toward the axillary line while remaining separated medially from the gastric impression by the organ’s thickness and the implied position of the hilum on the visceral surface. Superior and posterior margins read as the longest arcs in this dorsal profile, defining the splenic contour that lies deep to ribs 9 to 11. That contour matters when you are localizing the spleen behind the left lung base and pleura, because diaphragmatic surface injuries usually track with lower rib fractures and can produce subcapsular hematoma even without an obvious laceration on initial assessment. A clean posterior reference also helps in teaching why splenomegaly expands inferomedially into the abdomen, while the diaphragmatic surface remains constrained by the diaphragm and adjacent thoracic cage, a relationship that influences palpation findings and the expected direction of enlargement.
