Description
Oriented from an inferior perspective in the adult male, the liver is presented with emphasis on the diaphragmatic surface (facies diaphragmatica hepatis), its smooth convexity sweeping superiorly toward the domes of the diaphragm and tapering to the inferior border. The right lobe dominates laterally, while the left lobe extends medially toward the epigastrium, meeting at the anterior margin where the falciform ligament would attach on the anterior surface. Along the posterior-superior aspect, the bare area lies adjacent to the inferior vena cava groove, with the coronary ligament margins implied by the transition from peritoneal to nonperitoneal surface. This orientation matters when teaching how the liver seats against the diaphragm and why subdiaphragmatic pathology can refer pain to the shoulder via phrenic nerve afferents. It also supports surgical planning for mobilization: dividing the coronary and triangular ligaments to rotate the liver anteriorly exposes the retrohepatic inferior vena cava and short hepatic veins, a step that can make or break bleeding control. A clean read of the diaphragmatic contour helps correlate surface anatomy with operative findings during open or laparoscopic approaches to the right upper quadrant.
