Description
Posteriorly oriented, the liver is presented with emphasis on its superior (diaphragmatic) surface and the convex contour that abuts the diaphragm beneath the right hemidiaphragm. The posterior border rounds toward the bare area, where the hepatic peritoneum reflects and leaves a nonperitoneal patch that sits directly against the diaphragm. Medially, this posterior superior aspect leads toward the region of the inferior vena cava groove and the caval opening in the diaphragm, while laterally the dome of the right lobe extends toward the costal margin. A posterior presentation like this matters when you are teaching the difference between the diaphragmatic and visceral surfaces and explaining why the bare area behaves differently in trauma and infection. Subphrenic and subhepatic collections can track along peritoneal reflections, but the bare area creates a direct route for inflammatory spread to the diaphragm and pleura, contributing to right shoulder pain via phrenic nerve irritation. Small details in the posterior contour also anchor discussions of hepatic mobilization, since dividing the coronary and triangular ligaments around the bare area is central to many open and laparoscopic liver approaches.
