Description
Seen from an anterior angle, the diaphragmatic (superior) surface of the male liver fills the right upper quadrant beneath the dome of the diaphragm, with the convex right lobe extending farther laterally and superiorly than the left. The falciform ligament runs in a parasagittal plane from the anterior abdominal wall toward the liver, marking the surface division between the right and left lobes. Near its free inferior margin, the falciform ligament continues as the round ligament of the liver (ligamentum teres hepatis), a fibrous remnant that courses toward the umbilicus. For teaching surface anatomy, this orientation clarifies why the liver’s superior surface is largely nonperitoneal where it apposes the diaphragm and why the lobar division seen on the diaphragmatic surface does not match the functional (Couinaud) segmentation used in hepatobiliary surgery. It also anchors discussion of perihepatic spaces: fluid can collect between liver and diaphragm in the right subphrenic recess, and inflammatory spread can track along reflections of the falciform ligament. A clean external landmark. Use this illustration in gross anatomy courses when introducing peritoneal ligaments and hepatic surfaces, and in surgical atlases or review articles that contrast anatomical lobes with portal-based functional anatomy prior to hepatectomy.
