Description
Posteriorly, the trachea is presented with emphasis on the pars membranacea, the noncartilaginous wall that completes the C shaped tracheal rings. A band of trachealis muscle runs transversely within this membranous portion, spanning between the free posterior ends of the hyaline cartilages on the right and left. Immediately posterior to the membranous wall lies the esophagus, separated by loose connective tissue, while the cartilaginous arches occupy the anterior and lateral aspects of the airway. This is the compliant side. Recognition of the posterior tracheal membrane matters whenever the airway is manipulated. Excessive cuff pressure in an endotracheal tube or tracheostomy can injure the pars membranacea and adjacent esophageal wall, predisposing to tracheoesophageal fistula, and a weakened membranous segment can contribute to excessive dynamic airway collapse and tracheomalacia during forced expiration. Surgeons also use the posterior wall as a reference when repairing posterior tracheal lacerations or when mobilizing the trachea in the setting of proximal esophageal surgery. Orientation here drives safe dissection.
