Description
Posteriorly oriented, the left main bronchus is seen giving rise to the left superior lobar (secondary) bronchus (bronchus lobaris superior sinister), which courses superolaterally into the upper lobe before dividing into its early segmental bronchi. Branching occurs just distal to the carina, with the superior lobar bronchus positioned anterior to the pulmonary artery in the classic left-sided relationship, even when appreciated from a rear projection. Segmental trunks for the apicoposterior and anterior segments, along with the lingular division, separate as discrete luminal pathways. Clean geometry. Posterior placement matters when you need to teach or plan the bronchial route to the left upper lobe without the visual interference of cardiac mediastinal contours that dominate anterior views. This angle tracks how secretions, aspirated material, or an obstructing endobronchial lesion can preferentially involve upper-lobe segments, a pattern that changes the expected distribution of post-obstructive atelectasis and pneumonia. It also aligns with bronchoscopy orientation when sampling left upper-lobe lesions, where early segmental takeoffs can be missed if the superior lobar bronchus is not squared in the scope.
