Description
Superior to the left main bronchus, the upper lobar bronchus (bronchus lobaris superior sinister) branches laterally toward the superior lobe of the left lung, typically giving rise to segmental bronchi for the apicoposterior and anterior segments before coursing into the parenchyma. Medial to this airway axis lies the left pulmonary artery at the hilum, while the superior pulmonary vein tends to sit more anterior and inferior as it drains the upper lobe toward the left atrium. Cartilaginous rings and bronchial mucosa define the proximal bronchus, then taper as the airway divides distally. Airway anatomy is asymmetric. Understanding the left upper lobar bronchus matters in bronchoscopy and thoracic surgery because the left side lacks a bronchus intermedius and the branching pattern is tighter at the hilum, so minor malpositioning of an endotracheal tube can preferentially ventilate or obstruct the upper lobe. This view also supports interpretation of left upper lobe atelectasis or postobstructive pneumonia, where a mucus plug or an endobronchial lesion at the lobar origin can collapse the lobe and shift fissures and vascular markings on chest radiography or CT. Lymph nodes at the left hilar and interlobar stations often track disease spread along these bronchi.
