Description
Arising from the inferomedial aspect of the pancreatic head (caput pancreatis), the uncinate process projects medially and posteriorly in a hook-like contour. From a posterior orientation, its parenchyma lies posterior to the superior mesenteric vessels, with the superior mesenteric artery and vein running in the groove between the uncinate process and the neck of the pancreas. Inferiorly, the uncinate approaches the third part of the duodenum, while superiorly it blends into the head adjacent to the distal common bile duct as it courses toward the ampulla. A tight space. Posterior appreciation of the uncinate matters because it frames the key vascular relationships that govern resectability and operative risk in pancreatic head malignancy. In pancreaticoduodenectomy (Whipple) and uncinate-first approaches, surgeons work along the superior mesenteric artery plane to clear the mesopancreas and ligate venous tributaries, where small branches to the superior mesenteric vein and first jejunal vein can drive troublesome bleeding. This angle also supports teaching of tumor staging, since uncinate adenocarcinoma often presents with early SMA/SMV abutment and may not cause early biliary obstruction compared with more superior head lesions.
