Description
Arising from the inferior aspect of the pancreatic head, the uncinate process (processus uncinatus pancreatis) projects posteriorly and medially like a hook, draping behind the superior mesenteric vessels. From an inferior perspective, the superior mesenteric vein typically lies to the right of the superior mesenteric artery, with both structures coursing anterior to the uncinate process as they ascend toward the pancreatic neck. The lesser curvature side of the head blends superiorly into the pancreatic head proper, while the uncinate remains the most posterior and inferior portion of the gland. A tight corridor. Surgical anatomy here is unforgiving because this is where the SMA and SMV pass in close apposition to pancreatic parenchyma, so uncinate dissection during pancreaticoduodenectomy often defines the margin status. In pancreatitis or pancreatic head carcinoma, edema and tumor extension into the uncinate can tether or encase the mesenteric vessels, a pattern that influences resectability and the need for venous reconstruction. This angle also helps clarify why aberrant arterial anatomy, such as a replaced right hepatic artery from the SMA, becomes relevant during mobilization of the pancreatic head.
