Description

Posteriorly, the retroperitoneal (hidden) portions of the duodenum lie fixed against the posterior abdominal wall, with the descending (second), horizontal (third), and ascending (fourth) parts forming a C shaped frame around the pancreatic head. The third part crosses the midline anterior to the abdominal aorta and inferior vena cava, while the fourth part ascends to the left of the aorta to meet the jejunum at the duodenojejunal flexure, suspended by the ligament of Treitz. Proximally, the duodenum transitions from the duodenal bulb, but only its distal segments remain largely immobile because of their retroperitoneal attachments. This is the tethered gut. Seeing the duodenum from behind matters because these segments are the ones you mobilize, and sometimes injure, during retroperitoneal exposure. Kocherization releases the second part to access the pancreatic head, distal common bile duct, and inferior vena cava, while mobilization of the duodenojejunal flexure is a standard step in left sided vascular procedures and in approaching the origin of the superior mesenteric artery. The relationship of the third part to the aorta and vena cava also explains why pathology in the root of the mesentery or bulky nodes can distort the duodenal sweep, and why posterior duodenal ulcer disease can penetrate toward major vessels. Use this posterior view for gross anatomy teaching on retroperitoneal versus intraperitoneal bowel, and for surgical atlases covering the Kocher maneuver, SMA exposure, or duodenal trauma repair.
