Description
Proximal duodenum is presented from a posterior perspective, with the duodenal bulb (superior part, D1) running from the pylorus toward the right, then curving to meet the descending duodenum at the superior duodenal flexure. The posterior surface lies immediately anterior to the gastroduodenal artery and common bile duct in the hepatoduodenal region, while the anterior surface faces the visceral peritoneal cavity and the undersurface of the liver. Medially, D1 approaches the head of the pancreas, and its concavity frames the entrance into the C loop. Orientation is unmistakable. Clinically, the posterior wall of the duodenal bulb is the classic site where a peptic ulcer can erode into the gastroduodenal artery, producing brisk upper gastrointestinal hemorrhage. This angle also supports teaching of why pain from proximal duodenum can mimic gastric pathology and why posterior perforation may present with retroperitoneal signs once inflammation tracks near the pancreas. For surgeons and endoscopists, the pyloroduodenal junction is a key landmark when navigating the gastroduodenal region and anticipating the location of the hepatoduodenal ligament and its contents.
