Description
Posteriorly oriented anatomy centers on the superior duodenal flexure at the duodenojejunal angle (angulus duodenojejunalis), where the ascending (fourth) part of the duodenum turns anteroinferiorly to continue as proximal jejunum. Just superior and slightly posterior to the flexure, the suspensory muscle of the duodenum (ligament of Treitz) anchors the junction toward the right crus region and blends with the adjacent retroperitoneal connective tissue. The flexure lies left of the midline, with the pancreatic head and uncinate process positioned more to the right and anterior, and the posterior abdominal wall providing a fixed reference plane behind the bowel. Mesenteric root orientation becomes apparent here: mobile jejunum transitions away from the relatively fixed duodenum. This posterior perspective matters because the duodenojejunal junction is a repeatable landmark when you need to distinguish intraperitoneal small bowel from retroperitoneal duodenum, or when teaching why the ligament of Treitz defines the boundary between upper and lower gastrointestinal bleeding. Malrotation and internal hernia patterns often declare themselves at this flexure, and an abnormal position of the duodenojejunal angle is a key diagnostic criterion on contrast studies and CT. Misidentifying the junction can derail operative orientation during exploration for obstruction. A small but consequential point.
