Description
Arcing to the right of the midline in the upper abdomen, the first (superior) part of the duodenum transitions into the second (descending) part at the superior duodenal flexure. The bend sits inferior to the liver and gallbladder and anterior to the portal structures, while remaining closely applied to the head of the pancreas medially. Proximal continuity with the pylorus places the duodenal bulb superior and slightly anterior to the pancreatic head, with the descending segment turning inferiorly along its right margin. An anterior perspective prioritizes the curvature and its relationship to nearby hepatobiliary landmarks. Surgeons and endoscopists care about this angle because it is a consistent wayfinding point when moving from stomach to duodenum, and it frames common disease sites in the proximal small intestine. Peptic ulcer disease often targets the duodenal bulb near this transition, and posterior ulcers in this region can erode the gastroduodenal artery with brisk hemorrhage. Orientation at the superior duodenal flexure also helps contextualize edema or scarring that narrows the lumen, as well as deformity from inflammation in adjacent pancreas.
