Description
Centered within the abdominal cavity, the small intestine occupies most of the umbilical and hypogastric regions as tightly packed loops shown through a translucent anterior abdominal wall. Proximally, the duodenum begins just inferior to the stomach and sweeps toward the patient’s right before turning left at the duodenojejunal flexure, where the jejunum continues as thicker-caliber, more proximal coils. Distally, the ileum forms finer loops that trend inferiorly and toward the right lower quadrant, approaching the ileocecal junction. Mesenteric support is implied by the fan-like distribution of the loops from a relatively fixed root. This anterior orientation is the one clinicians mentally map during abdominal examination and cross-sectional imaging, because the small bowel shifts freely within the peritoneal cavity while key transition points remain comparatively constant. The duodenojejunal junction anchors at the ligament of Treitz, a landmark used to localize upper gastrointestinal bleeding, malrotation, and to separate foregut from midgut derivatives in teaching. Bowel obstruction often declares itself by proximal dilation and distal decompression, and this type of global, proportional rendering helps learners understand why jejunal loops tend to sit more left and central while the ileum commonly occupies the lower abdomen and pelvis.
