Description
Anterior anatomy of the proximal small intestine is presented with the duodenum forming its characteristic C-shaped loop around the head of the pancreas, the first (superior) part lying most superiorly and the second (descending) part running inferiorly along the right side before turning medially into the horizontal part. Superior and slightly right of midline, the extrahepatic biliary tree is traced as the common hepatic duct joins the cystic duct to form the common bile duct, which descends posterior to the first part of the duodenum toward the posteromedial wall of the second part. The distal bile duct aligns with the main pancreatic duct at the hepatopancreatic ampulla (ampulla of Vater) and enters the duodenum at the major duodenal papilla, a small but decisive landmark. This is the view you want when teaching or planning around biliary obstruction and periampullary disease. A stone impacted in the distal common bile duct can block bile flow and, by obstructing the shared channel, precipitate gallstone pancreatitis, a relationship that makes the terminal duct course and papilla location clinically non-negotiable. For operative anatomy, the illustration supports discussion of endoscopic retrograde cholangiopancreatography (ERCP), sphincterotomy at the sphincter of Oddi, and the way duodenal folds can hide the papilla from a straight-on approach.
