Along the anterior border of the left lung, the cardiac notch (incisura cardiaca pulmonis sinistri) interrupts the otherwise convex costal surface, creating a concavity that faces anteromedially toward the pericardium and left ventricle. Superior to this indentation lies the anterior segment of the upper lobe, while inferiorly the margin transitions toward the upper portion of the lower lobe along the oblique fissure region. The lingula of the upper lobe forms the inferior “tongue-like” extension that drapes anteroinferior to the notch, approaching the cardiophrenic region at the lung base. Clear contours of the left anterior (sharp) margin and adjacent mediastinal surface define the recess. Clinically, the cardiac notch and lingular margin explain why the heart produces a larger area of relative cardiac dullness on the left anterior chest and why the lingula can mimic right middle lobe behavior in localization of pneumonia. On axial CT and in left parasternal or anterolateral thoracic approaches, this indentation frames the pericardial contour and helps orient the reader to the left cardiophrenic angle, where a pericardial fat pad, pericardial effusion, or lingular atelectasis can alter expected silhouettes. Small changes matter. This is a landmark for correlating lung anatomy with chest radiography and cross-sectional imaging.
