Description
Anterior sectional anatomy through the apex cordis exposes the tapering left ventricle as it forms the cardiac tip, with the right ventricle lying more anterior and slightly superior to the apical myocardium. Along the anteroinferior contour, an apical indentation is identified as the incisura apicis cordis (apical notch), a shallow notch at the ventricular tip where the external contour subtly changes. The interventricular septum appears medial to the left ventricular free wall, and the overlying visceral pericardium and pericardial cavity margin the epicardial surface. A clean anterior section at the apex matters because it helps separate true apical morphology from artifacts created by obliquity in imaging planes or by partial volume effects on CT and cardiac MRI. The incisura apicis cordis is a useful surface landmark when correlating the external ventricular contour with internal chamber geometry, including apical trabeculations and the left ventricular cavity taper, which becomes relevant when assessing apical hypertrophic cardiomyopathy, left ventricular apical thrombus after anterior myocardial infarction, or suspected apical aneurysm. Small differences in where the section intersects the apex can change whether the notch is visible. That changes interpretation.
