Description
Oriented from an anterior sectional perspective, the apex cordis appears as the inferolateral, pointed termination of the heart, formed predominantly by the left ventricle with a smaller contribution from the right ventricle along the anterior surface. Superior to the apex, the left ventricular chamber wall thickens as it approaches the interventricular septum, while the right ventricle lies more anterior and medial in the section. A thin rim of pericardium and epicardial fat typically overlies the ventricular myocardium near the tip. Clear left sided dominance. Clinically, this is the landmark for the apical impulse, normally palpated in the left 5th intercostal space at or just medial to the midclavicular line, and its displacement inferolaterally is a classic bedside clue to left ventricular dilatation. The apical region also matters in imaging and intervention: transthoracic echocardiography uses apical windows to evaluate left ventricular function and the apex is a common site for mural thrombus after an anterior myocardial infarction. Surgeons and cardiologists reference the same geometry when planning apical access for left ventricular venting or selecting an entry trajectory that avoids the left anterior descending artery and its diagonal branches on the anterior surface. Ideal for teaching ventricular topography in gross anatomy and cardiology modules, or for illustrating the physical exam correlation between surface anatomy and left ventricular morphology in clinical skills texts and patient assessment guidelines.
