Description
Angled from an anterior perspective, the heart is presented with emphasis on the cardiac apex (apex cordis), the inferolateral, conical tip formed predominantly by the left ventricle. Superior to the apex, the anterior (sternocostal) surface is outlined by the right ventricle with the left ventricle contributing along the left cardiac border. The apex projects inferiorly and to the left relative to the base of the heart and great vessels, with the anterior interventricular sulcus descending toward it as a surface landmark for the left anterior descending artery (anterior interventricular branch). Clinically, the apex is the reference point for the point of maximal impulse and for auscultation over the mitral area, typically in the left 5th intercostal space near the midclavicular line. Small shifts matter. Left ventricular dilation, cardiomegaly, or a left pleural effusion can displace the apical impulse laterally and inferiorly, while dextrocardia or mediastinal shift can alter expected surface anatomy entirely. This anterior emphasis also supports teaching the distinction between the anatomical apex and the radiographic “cardiac apex” contour on chest X ray, where the left ventricular silhouette dominates the left heart border. Use this illustration in gross anatomy and cardiothoracic modules to anchor surface anatomy teaching, in clinical skills manuals covering cardiac examination, or in cardiology texts discussing PMI localization and left ventricular remodeling.
