Superior to the apex, the left ventricular myocardium rises toward the obtuse margin, and the pericardial investment would lie superficial to the epicardium along the diaphragmatic surface. Spatially, the apex sits inferior and lateral relative to the base of the heart, projecting toward the left 5th intercostal space in typical adult topography.
