Description
Anterior and leftward oblique positioning brings the sternocostal (anterior) surface of the cor into full relief, with the right ventricle occupying most of the anterior field and the left ventricle forming the left cardiac border. Superiorly, the base of the heart is suggested by the proximal great vessel origins, where the ascending aorta lies posterior and slightly right of the pulmonary trunk as they emerge from the ventricular outflow tracts. The anterior interventricular sulcus descends toward the apex, marking the boundary between ventricles and providing a clear longitudinal landmark. A simple, orientation-forward view. Clinically, this projection aligns with how the heart is approached and conceptualized in median sternotomy and in anterior thoracic imaging, where surface morphology guides rapid identification of ventricular dominance and border contours. The left cardiac border, shaped largely by left ventricle, is the contour that changes with left ventricular enlargement and pericardial effusion on chest radiography, while the anterior interventricular groove approximates the course of the left anterior descending coronary artery, a common site of occlusion in acute coronary syndrome. Small shifts in contour matter. This is where they start.
