Description
Oblique anterior-left orientation exposes the sternocostal (anterior) surface of the heart as a continuous expanse of myocardium, with the right ventricle forming most of the anterior wall and the left ventricle contributing the left (sinistral) border and apex. Superiorly, the anterior interventricular sulcus can be appreciated as a surface landmark between the ventricles, even though the anterior interventricular vessels are intentionally absent. The right atrium lies to the viewer’s right in anatomical position, while the left auricle typically drapes anterior to the left atrium near the base. Removing the coronary arteries and veins keeps attention on ventricular contours and external topography, which is often what learners and clinicians need when correlating the physical heart to radiographs, pericardial relationships, or operative exposure. This is the surface encountered in median sternotomy and many pericardiotomy openings, where the right ventricle is the first chamber in the field and is vulnerable to injury in re-entry. Clean myocardial planes also help teach why the left anterior descending artery normally tracks the anterior interventricular sulcus and how infarct territories are discussed even when vessels are not drawn.
