Description
Seen from a lateral aspect, the cardiac wall is organized into epicardium (visceral layer of serous pericardium) externally, myocardium as the thick middle muscular layer, and endocardium lining the chambers internally. Along the left cardiac border, the left ventricle forms the dominant contour, with the left atrium positioned superior and posterior to it and the apex directed inferiorly and to the left. Coronary vessels and their branches typically course within the epicardial fat on the external surface, while myocardial fibers beneath them wrap in oblique and circumferential bundles that thicken toward the ventricular free wall. Lateral myocardium is the tissue that fails in left ventricular hypertrophy and remodels after anterolateral myocardial infarction, so this angle aligns well with the clinical language used in echocardiography and cardiac MRI reporting of lateral wall motion abnormalities. Orientation matters. Surgeons also rely on the relationship between epicardial serosa and underlying muscle when dissecting in the atrioventricular and interventricular grooves, where the coronary arteries and cardiac veins lie superficial to the myocardium but deep to the pericardial reflections.
