Description
Left ventricular myocardium forms the dominant muscular layer of the left heart wall in this lateral view, positioned deep to the epicardium (visceral pericardium) and superficial to the endocardium lining the ventricular cavity. Along the lateral (obtuse) margin, myocardial thickness is greatest and wraps from the anterior wall toward the posterolateral wall, continuous superiorly with the muscular left atrium and inferiorly with the ventricular apex. Epicardial fat and the course of the left coronary arterial branches can be inferred on the surface, while the endocardium remains the smooth inner boundary. Orientation is clear. Left is left. That wall thickness matters clinically because the left ventricular myocardium drives systemic ejection and is the substrate for hypertrophic remodeling in long-standing hypertension and aortic stenosis. Lateral wall ischemia and infarction often track the obtuse marginal branches of the circumflex artery, and a side-on view helps correlate epicardial coronary territories with the underlying myocardial layer that becomes hypokinetic on echocardiography. The same anatomic plane also supports teaching of transmural versus subendocardial injury patterns, where perfusion gradients make the inner myocardium more vulnerable during demand ischemia. Use this asset in cardiovascular anatomy and physiology courses when introducing the heart wall layers (endocardium, myocardium, epicardium) and in pathology texts illustrating left ventricular hypertrophy, myocardial infarction distribution, or ventricular remodeling.
