Description
Oriented in a medial (mediastinal) view, the left lung is presented with emphasis on the superior (apical) segment of the lower lobe, typically referenced as segmentum superius (S6) in bronchopulmonary segmental anatomy. The vertebral, or dorsal, surface is dominant, with the posterior convexity conforming to the thoracic vertebral column and the paravertebral gutter. Superiorly, the apex rises above the level of the first rib, while inferiorly the diaphragmatic surface slopes toward the base; the segmental coloration isolates S6 against adjacent lower lobe parenchyma. Segmentum superius matters because it sits at the junction of lobar and paravertebral anatomy where airways and vessels follow predictable but surgically relevant patterns. This segment receives the superior segmental bronchus (B6) and accompanying segmental pulmonary artery branch, a relationship used when planning segmentectomy for small peripheral non small cell lung cancers or targeted resection for localized bronchiectasis. Posterior, dependent positioning also explains why aspiration and postoperative atelectasis often favor dorsal basal segments in supine patients, while S6 remains a key landmark when correlating CT findings along the oblique fissure and the vertebral surface.
