Description
Arising from the superior lobe of the right lung, the apical segment (segmentum apicale, S1) is isolated in color and presented in a medial view from a posterior approach, so its pleural surface sits immediately deep to the posterior thoracic cage. Superiorly it extends toward the cupola beneath the first rib and clavicle, while inferiorly it tapers toward the posterior portion of the horizontal fissure region and the adjacent posterior segment of the upper lobe. Medially, the segment’s hilum-facing surface relates to the right main bronchus as it gives rise to the upper lobar bronchus and the segmental bronchus to S1, with accompanying branches of the pulmonary artery and veins tracking along intersegmental planes. Clear borders matter here. Segmental anatomy of the superior lobe is not academic trivia when you are planning a segmentectomy or interpreting a CT in a patient with a solitary pulmonary nodule at the apex. The apical segment is a common site for cavitary disease and post-primary tuberculosis, and its posterior adjacency to the thoracic wall explains why apical pleural thickening and adhesions can complicate video-assisted thoracoscopic access. This view also helps teach why a Pancoast tumor at the lung apex often presents with neurovascular symptoms even before it produces obvious endobronchial findings. Use this artwork in thoracic surgery teaching files to orient residents to right upper lobe segmental targets, in radiology atlases that map bronchopulmonary segments for reporting, or in pulmonary medicine coursework covering lobar and segmental localization of infection and neoplasm.
