Description
Arising from the inferior trachea (windpipe), the right and left primary (main) bronchi diverge at the carina in the midline, forming the first bifurcation of the bronchial tree. Each main bronchus courses inferolaterally toward its respective lung hilum, with the right main bronchus typically shorter, wider, and more vertical than the left. At the tracheobronchial junction, the cartilaginous support transitions from the C-shaped tracheal rings to irregular bronchial cartilage plates as the airway begins its intrapulmonary branching pattern. Orientation is anchored by the carina, positioned inferior to the tracheal lumen and superior to the proximal main bronchi. Carinal and main bronchus anatomy matters because it is the key endoscopic and radiographic landmark for airway patency and tube placement. A right mainstem intubation occurs because the right primary bronchus aligns more directly with the trachea, leading to unilateral ventilation and left lung atelectasis if unrecognized. The carina itself is sensitive to distortion from subcarinal lymphadenopathy, central bronchogenic carcinoma, or extrinsic compression, and bronchoscopy often uses its sharp ridge to confirm midline orientation before advancing into lobar bronchi.
