Seen in lateral profile, the adult male cecum (caecum) forms a blind-ended sac in the right iliac fossa, positioned inferior to the ileocecal junction where terminal ileum enters the large intestine. The ileocecal valve lies at the superomedial aspect of the cecal lumen, while the ascending colon continues superiorly from the cecum along the right flank. Anterolaterally, the cecal wall approaches the parietal peritoneum, with the posterior surface typically relating to the iliacus and psoas major region. Orientation matters here. This lateral location is the practical backdrop for right lower quadrant pain workups and for planning access during appendectomy, since the vermiform appendix most often arises from the posteromedial cecum just inferior to the ileocecal valve, even when its tip runs retrocecal. A profile view also supports teaching about mobility: the cecum can be variably peritonealized and mobile, and malrotation or an unusually mobile cecum can shift expected surface anatomy and alter where tenderness localizes.
