Description
Running in a deep vertical sulcus on the posterior aspect of the liver, the groove for the inferior vena cava (sulcus venae cavae) lies to the right of the caudate lobe and immediately posterior to the porta hepatis. Its cranial end approaches the bare area before the vessel enters the diaphragm at the caval opening, while caudally it continues toward the hepatic veins and the retrohepatic segment of the inferior vena cava. Medially, the caudate process bridges toward the right lobe, and laterally the groove abuts the visceral surface of the right hepatic lobe. Surgeons care about this relationship because the retrohepatic inferior vena cava is fixed to the liver by short hepatic veins and fibrous attachments, making mobilization of the right lobe hazardous when bleeding starts. That matters in hepatectomy, trauma packing, and liver transplantation, where control of the hepatic veins at their junction with the inferior vena cava can be the difference between a controlled field and rapid exsanguination. For anatomy teaching, the sulcus venae cavae also anchors orientation of the posterior liver when the classic anterior landmarks are out of sight. A hard landmark. Use this illustration in gross anatomy and surgical anatomy curricula to teach posterior hepatic surface orientation, and in operative atlases or transplant manuals to clarify the plane between the liver capsule and the retrohepatic inferior vena cava during right lobe mobilization and caval control. It also fits well in radiology teaching files when correlating posterior liver anatomy with CT or MRI views of the retrohepatic inferior vena cava and hepatic vein confluence.
