Description
Seen from a superior orientation, the diaphragmatic surface of the male liver is presented with the sulcus for the inferior vena cava (sulcus venae cavae) cut as a deep vertical groove in the posterior part of the right lobe. The groove runs superiorly toward the bare area, lying posterior to the fossa for the gallbladder and lateral to the fissure for the ligamentum venosum as it approaches the hepatic venous confluence. Close by, the right and middle hepatic veins course toward their ostia where they drain directly into the inferior vena cava near the superior border of the organ. A posterior landmark. For operative anatomy, this is the piece of surface topography that tells you where the retrohepatic inferior vena cava is fixed to the liver parenchyma, and where short hepatic veins can complicate mobilization. During hepatic resection or the piggyback technique in liver transplantation, dissection along the bare area and around the caval groove determines how safely the liver can be rotated off the vena cava without avulsion of those small, thin walled tributaries. It also clarifies why Budd-Chiari syndrome and hepatic venous outflow obstruction concentrate clinically at the junction of hepatic veins and the caval segment. Use this plate when teaching peritoneal reflections and hepatic surface anatomy in gross anatomy and surgical anatomy courses, or when illustrating retrohepatic IVC exposure in hepatobiliary surgery and transplant manuals. It also fits radiology teaching files that correlate the caval groove and hepatic venous drainage with axial CT or MR landmarks at the superior liver.
