Description
Viewed from a superior aspect, the pancreatic tail tapers laterally from the pancreatic body toward the splenic hilum, lying in the left upper quadrant along the splenic capsule. Anterior to the gland sit the posterior wall of the stomach and the lesser sac, while the tail remains posterior to the stomach and inferior to the splenic artery as it courses along the superior border of the pancreas. The left kidney and left suprarenal gland occupy a more posterior plane, and the splenic flexure of the colon lies inferiorly. A narrow end segment. For pancreatic surgery, this superior reference point matters because the tail is the portion most closely related to the splenic vessels and the splenic hilum, where a few millimeters can separate safe dissection from vascular injury. During distal pancreatectomy, surgeons often work along the superior border to identify and control the splenic artery and vein, and this view clarifies how the tail can extend into the splenorenal ligament and meet the spleen without a clear fascial plane. It also supports interpretation of CT or MRI in cases of tail pancreatitis or adenocarcinoma, when posterior gastric impression and proximity to the spleen can complicate staging and resectability.
