Description
Anteriorly oriented, the expanded right end of the pancreas is presented as the head region (often colloquially called the base), seated in the C loop of the duodenum and continuous medially with the pancreatic neck and body. Superior to the head, the pylorus and first part of the duodenum lie in close apposition, while the inferior margin approaches the horizontal (third) part of the duodenum. Posterior relations are implied by the contours of the gland, with the expected course of the common bile duct in a groove on the posterolateral aspect of the head and the uncinate process projecting posteriorly and medially. Clinical anatomy in this region is unforgiving. Carcinoma of the pancreatic head commonly presents with painless obstructive jaundice because even small lesions can narrow the distal common bile duct, and understanding the head’s relationship to the duodenum helps explain early gastric outlet symptoms when the periampullary region is involved. For operative planning, this anterior perspective supports teaching the landmarks used in pancreaticoduodenectomy (Whipple procedure), including where the neck crosses the portal vein confluence and why the uncinate process dissection tracks along the superior mesenteric vessels.
