Description
Positioned on the superomedial poles of the kidneys, the paired suprarenal (adrenal) glands sit retroperitoneally against the posterior abdominal wall, with the adrenal cortex forming an outer rim around the deeper medulla. From a posterior perspective, their relationship to the upper renal poles is clear, as are the vertically oriented abdominal aorta (left of midline) and inferior vena cava (right of midline) with the renal arteries and renal veins coursing laterally into the renal hila. Ureters descend inferiorly from each kidney, while small yellow elements adjacent to the great vessels suggest autonomic nerves and or regional lymphatic tissue. The adrenal glands read as distinct caps, superior to the kidneys and medial to the upper pole contours. Posterior anatomy matters here because the suprarenals are not “on top of” the kidneys so much as they are anchored to the diaphragmatic crura and adjacent musculature, a relationship that influences how masses displace nearby structures. This view supports teaching of adrenal vascular anatomy: multiple small superior, middle, and inferior suprarenal arteries can arise from the inferior phrenic artery, abdominal aorta, and renal artery, while venous drainage is asymmetric, the right suprarenal vein draining directly to the inferior vena cava and the left suprarenal vein typically draining to the left renal vein. That asymmetry is a recurring operative and imaging pitfall. Small vein, big consequences.
