Description

Prominent on the medial thigh, the adductor magnus spans from the pelvis to the femur, filling the space posterior to the adductor longus and adductor brevis and anterior to the hamstring compartment. Its adductor part arises from the inferior ramus of the pubis and ramus of the ischium and runs inferolaterally to the linea aspera and medial supracondylar line, while the hamstring part courses more posteriorly to a distal insertion on the adductor tubercle. Bony landmarks include the acetabulum and proximal femur at the hip joint, with the distal femur oriented toward the knee. Separating the adductor and hamstring portions is not academic. The hamstring part is innervated by the tibial division of the sciatic nerve, whereas the adductor part receives obturator nerve fibers, a split that helps explain mixed patterns of weakness after pelvic trauma or iatrogenic nerve injury. The distal tendon at the adductor tubercle forms the medial boundary of the adductor hiatus, the passageway where the femoral artery and vein transition to the popliteal vessels. A critical surgical landmark.
