Description
Running superficially along the medial side of the male lower limb, the great saphenous vein begins from the dorsal venous arch of the foot and courses anterior to the medial malleolus before ascending along the medial border of the tibia toward the knee. Proximal to the medial femoral condyle it continues up the anteromedial thigh, accompanied variably by the saphenous nerve, and approaches the groin where it passes through the saphenous opening of the fascia lata to join the femoral vein at the saphenofemoral junction. Multiple venous tributaries and venules converge along its length, while deeper veins track nearer the tibia, fibula, and femur beneath the muscular compartments. Clear separation of superficial and deep venous pathways matters when you are mapping varicose vein disease or planning intervention for venous reflux. The great saphenous vein is the usual culprit in primary varicosities, and the competence of the terminal and preterminal valves at the saphenofemoral junction drives both duplex ultrasound interpretation and the choice of endovenous ablation versus high ligation and stripping. Landmarks shown here, the medial malleolus, medial knee, and inguinal region over the femoral triangle, also align with common sites of phlebotomy access, vein harvesting for bypass, and iatrogenic saphenous nerve irritation during procedures.
